Well I thought I'd update this.

Briefly, one new GFX card later and my problems are gone. New card (same type) runs cooler and quieter and give no issues at all. Must have been a hardware fault.

Hopefully I've found a stable setup now. I removed all of the drivers and software that came with the card (Gigabyte stuff) and now I'm only running the direct AMD drivers and the AMD Catalyst Control Centre, with the GPU clock speed reduced by 5%. I'll look at any bios settings as I don't think my fans are running at high speed.

The company that supplied it to me are getting me to test it to try and see where the problem might be.
